The St. John Occupational Healthcare Process
Our process is simple and straightforward, making it convenient for businesses and their employees.
- Injured employee reports to supervisor.
- Supervisor completes Authorization to Treat form and directs employee to report to a St. John Occupational Healthcare (SJOHC) facility.
- Employee presents Authorization to Treat form at the SJOHC facility for diagnosis and treatment.
- SJOHC communicates the employee’s work status to employer.
- SJOHC generates a Physician’s First Report, and forwards a copy to employer representative and/or insurance carrier.
