The St. John Occupational Healthcare Process
Our process is simple and straightforward, making it convenient for businesses and their employees.

  1. Injured employee reports to supervisor.
  2. Supervisor completes Authorization to Treat form and directs employee to report to a St. John Occupational Healthcare (SJOHC) facility.
  3. Employee presents Authorization to Treat form at the SJOHC facility for diagnosis and treatment.
  4. SJOHC communicates the employee’s work status to employer.
  5. SJOHC generates a Physician’s First Report, and forwards a copy to employer representative and/or insurance carrier.
